What To Know Before You Visit

  • To make your first visit as efficient as possible, please consider downloading and completing the forms below prior to your appointment time
  • Bring your prescription from your provider and insurance card
  • Arrive at least 15 minutes prior to your first appointment
  • Wear comfortable, loose fitting clothing. Bring shorts for lower extremity injuries
  • Wear tennis shoes
  • Write down any questions you may have

What To Expect At Your First Visit

  • Your first visit will last approximately one to one and a half hours, depending on your condition
  • You will be evaluated by a licensed physical or occupational therapist
  • Your evaluation will include a thorough medical history and evaluation of range of motion, strength, mobility, and coordination, among others
  • We will collaborate with you on a plan that will return you to your prior level of function

Below are the forms needed prior to your first visit. While filling out paperwork is sometimes tedious and repetitive, it does come with a purpose. The information gathered improves our ability to fully assess your medical history, screen for potential serious medical conditions, and limit time spent asking questions. This allows us to spend our first session discussing your recovery and ways to improve your ability to return to the things you love.
