Below are the forms needed prior to your first visit. While filling out paperwork is sometimes tedious and repetitive, it does come with a purpose. The information gathered improves our ability to fully assess your medical history, screen for potential serious medical conditions, and limit time spent asking questions. This allows us to spend our first session discussing your recovery and ways to improve your ability to return to the things you love. Accurate completion of these forms also allow us to bill your insurance and prevent delays in authorization. Thank you in advance for your patience.
