What is the MCL?

MCL is an abbreviation for “medial collateral ligament.” A ligament is a tough, flexible of connective tissue that attaches bones to each other and provides stability. The MCL connects your upper shin bone to the inner surface of the bottom thigh bone, running along your inner knee, and maintains stability in the inner knee. Its functions to withstand valgus stress, or the knee being pushed too far inward. It is one of four ligaments that help control the knee, along with the:

  • ACL (anterior cruciate ligament), which goes through the middle of your knee and provides rotational stability and controls forward movement
  • PCL (posterior cruciate ligament), which crosses the ACL in the middle of the knee and prevents excessive posterior translation of the tibia relative to the femur
  • LCL (lateral collateral ligament), which connects the femur to the fibula (the smaller bone between the knee and ankle) and prevents the knee from being forced unnaturally outward

The knee is one of the most complex joints we have. It depends on surrounding bone and muscle, and the tendons and ligaments in order to provide support and function properly. When any knee injury, such as an MCL tear, occurs it can limit or completely restrict your mobility and ability to do what you love most, or just live a pain-free day-to-day life. Particular physical therapy exercises and other treatment modalities, prescribed by a doctor, have been demonstrated through research and analysis to help patients manage their symptoms and get back to normal functionality.

What Types of MCL Injuries Are There?

In general, an MCL injury either involves a sprain, which stretches the ligament beyond its normal range of motion, or a tear, where the MCL is either partially or completely torn or separated from the bone. There are three grades to MCL injuries:

  • Grade 1 — A grade 1 MCL injury refers to a sprain, or a tear where less than 10% of the MCL has been torn. It’s typically a mild injury, and your knee largely remains stable. Your knee will feel tender, and you may also experience soreness.
  • Grade 2 — This injury is when the MCL suffers more significant tearing, generally to the more fibrous part of the MCL. Your knee will be slightly unstable, and your joint may be loose when manipulated manually. A grade 2 injury will result in significantly more pain than a grade 2 MCL injury.
  • Grade 3 — This occurs when your MCL is entirely torn, both the superficial and deep parts. Your knee will be significantly unstable. Pain will be intense and you’ll likely feel a wide array of symptoms. Grade 3 MCL injuries often occur alongside other knee injuries such as ACL tears.

Whatever grade MCL injury you have, our qualified Fenton, MO physical and occupational therapists at Axes can, along with your physician, develop a comprehensive and tailored plan to get you back to the activities your love most safely.

What Causes MCL Injuries?

Ligament injuries account for 40% of all knee injuries, and the MCL is the most commonly injured ligament. When you suffer a forceful trauma to the outside of your knee, it can push the knee inside far enough that the MCL strains or tears. MCL injuries can also occur if the knee is rotated more than the ligament can stand. They can also take place when landing or unnatural bending, such as if a cleat is stuck in grass while your leg keeps moving.

Athletes in contact sports are the most likely to sustain an MCL injury. They are also common in those who have to squat and lift heavy objects, either for athletic training or in the course of their employment.

Common Symptoms of MCL Injuries

If you suffer an MCL injury in Fenton, MO, you’ll likely notice the pain right away. Other symptoms may include:

  • A sudden “pop” on the inside of your knee
  • Feeling like the may buckle
  • Not being able to put any weight on the knee
  • Bruising
  • Tenderness where the MCL is located
  • Swelling, either of the MCL area or the whole knee

Sometimes your knee may feel like your knee is loose when you walk, even if walking doesn’t hurt, while other times the knee feels too rigid. If you experience any of the above symptoms, do not participate in any more strenuous activity and seek medical treatment for a diagnosis.

Diagnosis and Treatment of an MCL Injury in Fenton, MO

Diagnosing an MCL injury requires going to a physical examination. In the examination, your Fenton, MO doctor or physical therapist will go over how the injury occurred, will feel for swelling and how your knee is different from your healthy knee, and gently see if your knee is unstanble while bending inward, and your pain level.

Most physical therapists or doctors that specialize in mobility of the human body in Fenton, MO will be able to diagnose an MCL injury with a physical examination. A phsycian may also order the following imaging tests to check the severity of the injury and whether or not you have suffered other damage to the knee:

  • An MRI (magnetic resonance imaging), which uses a magnet and radio waves to take detailed images of the inside of your knee
  • An ultrasound, which uses sound waves and can help determine the grade of your injury
  • An X-ray, which does not show soft tissue, to insure your femur, kneecap, tibia, and fibula have not been broken

You can also schedule and appointment with the Axes location nearest you for a complimentary, no-risk injury screening. While we will be able to diagnose your injury, Missouri law stipulates that physical therapy be prescribed by a doctor.

Treating an MCL Injury in Fenton, MO

Typically, treating an MCL injury begins with basic first aid: swelling reduction, reducing pain, and utilizing the R.I.C.E. Method of Rest, Ice, Compression, and Elevation. For grade 1 injuries, the R.I.C.E method, over-the-counter pain relievers, and a few days or week or two of rest may be all you need for your body’s natural healing processes to get you back on your feet.

Professional physical therapy is needed for grade 2 and 3 injuries to restore strength and function and get you back to racing down or around the court or field. You’ll also need time resting to allow your body to heal safely. Your Axes physical therapist in Fenton, MO will be able to fit you for a knee brace so that you don’t suffer further injury. With physical therapy, you’ll still continue icing your knee and taking anti-inflammatory pain medication.

Though rare, the most severe cases — in which the MCL is completely torn, the injury is accompanied by an ACL tear, or nonsurgical treatment just isn’t working — may require surgery. The surgery is carried out by using a graft, either from your hamstring or from another person, to reattach the torn sections. You would not have to be hospitalized overnight. If your doctor thinks surgery is necessary, our physical therapists specialize in pre- and post-surgical rehabilitation to make your surgery and recovery as successful as possible.
