How the MCL Works

MCL stands for “medial collateral ligament.” A ligament is a tough, flexible of connective tissue that connects bone to bone and provides stability. The MCL connects your upper shin bone to the inner surface of the bottom thigh bone, running along your inner knee, and helps keep your knee stable and functioning normally. Its functions to guard against valgus stress, or the knee being forced too far towards the other one. It combines with three other ligaments that work together to support normal function and stability:

  • ACL (anterior cruciate ligament), which goes through the middle of your knee and controls rotation and prevents the knee from bending inward
  • PCL (posterior cruciate ligament), which crosses the ACL inside the knee and the femur from sliding off the tibia
  • LCL (lateral collateral ligament), which runs along the outside of the knee from the femur to the fibula (the smaller bone in the lower leg) and prevents the knee from being forced unnaturally outward

The knee is one of the most complex joints in the human body. A healthy knee relies on surrounding bone and muscle, and the tendons and ligaments in order to provide stability and function properly. There are a variety of knee injuries that can limit or completely restrict your mobility and compete like you’re used to, or just complete basic tasks pain-free. Specific physical therapy techniques and other treatment modalities, prescribed by a doctor, have been demonstrated through evidence to help those who have suffered an MCL injury mitigate pain and swelling and return to optimal mobility.

What Types of MCL Injuries Are There?

For all intents and purposes, an MCL injury either involves a sprain, wherein the MCL extends beyond its normal range of motion, or a tear, where the MCL is either partially or completely torn or separated from the bone. There are three grades to MCL injuries:

  • Grade 1 — A grade 1 MCL injury refers to a sprain, or a tear where less than 10% of the the fibers in your MCL are torn. These injuries are typically not especially painful or debilitating, and don’t result in much instability. Symptoms will likely be tenderness and some soreness.
  • Grade 2 — This is more significant tearing, generally to the more fibrous part of the ligament. You will probably lose some stability in your knee, and the knee will likely be loose when moved by hand. Pain is much more severe in a grade 2 MCL injury.
  • Grade 3 — This injury is a complete tear to both the fibrous and deep parts. Your knee will be significantly unstable. Pain will be intense and you’ll likely feel several other symptoms. Grade 3 MCL injuries often occur alongside other knee injuries such as ACL tears.

However significant your MCL injuries, our professional Augusta, MO physical and occupational therapists at Axes can, as a team with your doctor, develop a comprehensive and personalized plan aimed at returning you to the life you’re used to without incurring further injury.

MCL Injuries: Causes and Symptoms

Ligament injuries account for 40% of all knee injuries, and most of the time that ligament is the MCL. Most MCL injuries occur due to physical trauma on the outside of the knee. MCL injuries can also occur if the knee is rotated too forcefully. They can also occur because of landing or awkward bending, such as if a cleat is stuck in grass while your leg keeps moving.

Athletes in contact sports are the most likely to sustain an MCL injury. They can also occur in people who lift weights or lift heavy objects at their work.

Common Symptoms of MCL Injuries

If you suffer an MCL injury in Augusta, MO, you’ll likely notice the pain right away. Other symptoms may include:

  • A sudden “pop” on the inside of your knee
  • Instability
  • Not being able to put any weight on the knee
  • Contusions
  • Tenderness on the inside of your knee
  • Swelling, either of the MCL area or the entire knee joint

Sometimes your knee may feel like your knee is loose when you walk, even if walking doesn’t hurt, while other times you’ll barely be able to move the knee. If you experience any of the above symptoms, cease any strenuous activity immediately and schedule a physical examination in order to get a diagnosis.

Diagnosing and Treating an MCL Injury in Augusta, MO

MCL injuries are diagnosed in a physical examination. The exam will usually start with a doctor or physical therapist in Augusta, MO asking how the injury occurred. They’ll then bend your knee and apply slight pressure to the outside of it, noting how much pain you are in and whether or not your knee is unstable. They’ll also gently feel for swelling, and may compare your injured knee to the healthy one.

Most physical therapists or doctors that specialize in mobility of the human body in Augusta, MO will be able to diagnose an MCL injury with the above procedures. A phsycian may also order the following imaging tests to check the severity of the injury and whether or not you have suffered other damage to the knee:

  • An MRI (magnetic resonance imaging), which uses a magnetic field and computer-generated radio waves to take three-dimensional images of the knee joint
  • An ultrasound, which uses sound waves and can be helpful in checking the severity of your injury
  • An X-ray, which, while it won’t show the MCL or other ligaments, may be ordered for a severe MCL injury to ensure there are no broken bones

You can also visit the Axes location nearest you for a free, no-obligation injury screening. While we will be able to diagnose your injury, Missouri Revised Statute §334.506 stipulates that physical therapy be prescribed by a doctor.

How MCL Injuries are Treated in Augusta, MO

Typically, treating an MCL injury begins with basic first aid: inflammation reduction, reducing discomfort, and using the R.I.C.E. Method of Rest, Ice, Compression, and Elevation. For mild injuries, the R.I.C.E method, pain and swelling relievers, and a few days or week or two of rest may be enough to get mostly back to normal.

Grade 2 or 3 tears will require professional physical therapy in order truly recover from your MCL injury and get back to the activities you love. You’ll also need to refrain from strenuous until your knee is strong again. Your Axes physical therapist in Augusta, MO can fit you for a knee brace so that you don’t suffer further injury. Even with physical therapy, you’ll likely still continue the R.I.C.E. method and anti-inflammatory medication.

Though uncommon, the most severe cases — in which the MCL is completely torn, the injury is accompanied by an ACL tear, or you are not recovering after several weeks of nonsurgical treatment — may require surgery. During the surgery, a surgeon will use a graft, either from your hamstring or an organ donor, to reconstruct the MCL. The surgery is an outpatient procedure. If you do require surgery in Augusta, MO, our physical therapists specialize in pre- and post-surgical rehabilitation get you the best possible results from your surgery.
