The Anatomy of the MCL

MCL an acronym for “medial collateral ligament.” A ligament is a fibrous of connective tissue that affixes bone to bone and keeps joints stable. The MCL connects your upper shin bone to the inner surface of the bottom thigh bone, running along your inner knee, and helps keep your knee stable and functioning normally. Its primary purpose is to guard against valgus stress, or the knee being pushed unnaturally towards the other one. It is one of four ligaments that help keep the knee stable, along with the:

  • ACL (anterior cruciate ligament), which goes through the middle of your knee and controls rotation and controls forward movement
  • PCL (posterior cruciate ligament), which crosses the ACL in the middle of the knee and the femur from sliding off the tibia
  • LCL (lateral collateral ligament), which runs along the outside of the knee from the femur to the fibula (the smaller bone the small bone behind the tibia) and helps keep the outer side of your knee joint stable

The knee is known for being one of our most complex joints. A healthy knee relies on surrounding bone and muscle, and the tendons and ligaments in order to provide stability and function properly. There are a variety of knee injuries that can limit or completely restrict your mobility and ability to do what you love most, or just live a pain-free day-to-day life. Particular physical therapy techniques and other physician-prescribed treatment plans have been proven to help patients mitigate pain and swelling and return to normal mobility.

What Types of MCL Injuries Are There?

For all intents and purposes, an MCL injury either involves a sprain, which stretches the ligament beyond its normal range of motion, or a tear, where the MCL may fray partially or entirely separate from the bone. There are three grades to MCL injuries:

  • Grade 1 — A grade 1 MCL injury refers to a sprain, or a tear where less than 10% of the MCL has been torn. These are not generally severe, and don’t result in much instability. Your knee will feel tender, and you may also experience soreness.
  • Grade 2 — This is more significant tearing, most commonly to the superficial part of the ligament. Your knee will be slightly unstable, and your joint may be loose when moved by hand. Pain is much more severe in a grade 2 MCL injury.
  • Grade 3 — A grade 3 tear is a complete tear, or separation from the bone. You will likely feel like your knee’s about to “give out.” You will likely experience significant pain and several other symptoms. The injury may actually caused by or accompanied by other serious knee injuries such as ACL tears.

However significant your MCL injuries, our professional Pevely, MO physical and occupational therapists at Axes can, alongside your doctor, implement an extensive plan uniquely made for you and your injuries to get you back to the things you love without incurring further injury.

MCL Injuries: Causes and Symptoms

Ligament injuries account for 40% of all knee injuries, and most of the time that ligament is the MCL. When you suffer a forceful trauma to the outside of your knee, it can push the knee inside far enough that the MCL strains or tears. MCL injuries can also occur if the knee is rotated too forcefully. They can also take place when landing or unnatural bending, such as if your foot stays in one place while your leg keeps moving.

Athletes in contact sports are the most likely to sustain an MCL injury. They are also common in those who have to hunch over and lift heavy objects, either for athletic training or in the course of their employment.

Common Symptoms of MCL Injuries

Most MCL injuries are accompanied by immediate, noticeable pain. Other symptoms may include:

  • Hearing or feeling a popping noise or feeling
  • Instability
  • Not being able to put any weight on the knee
  • Contusions
  • Tenderness where the MCL is located
  • Inflammation, either of the MCL area or the entire knee joint

Sometimes you may feel a “wobbly” sensation, or in other cases you’ll barely be able to move the knee. If you suspect you may have suffered an MCL injury, do not participate in any more strenuous activity and schedule a physical examination in order to get a diagnosis.

Diagnosis and Treatment of an MCL Injury in Pevely, MO

MCL injuries are diagnosed in a physical examination. In the examination, your Pevely, MO doctor or physical therapist will go over how the injury occurred, will feel for swelling and how your knee is different from your healthy knee, and gently see if your knee is unstanble while bending inward, and your pain level.

Most physical therapists or physicians that specialize in the musculoskeletal system in Pevely, MO are qualified to diagnose an MCL injury with a physical examination. Other tests to check severity or to confirm may include:

  • An MRI (magnetic resonance imaging), which uses a magnetic field and radio waves to produce detailed images of the knee joint
  • An ultrasound, which uses sound waves and can be helpful in checking the severity of your injury
  • An X-ray, which does not show soft tissue, to insure your femur, kneecap, tibia, and fibula have not fractured

You can also come to the Axes location most convenient for you for a complimentary, no-risk injury screening. While we can offer you an evaluation, Missouri Revised Statute §334.506 mandates that physical therapy be prescribed by a doctor.

How MCL Injuries are Treated in Pevely, MO

Right after your MCL injury, you should start with basic first aid: reducing swelling, relieving discomfort, and using the R.I.C.E. Method of Rest, Ice, Compression, and Elevation. For grade 1 injuries, the R.I.C.E method, over-the-counter pain relievers, and some time with limited activity may be sufficient for your body to heal itself.

More significant injuries will require professional physical therapy in order truly recover from your MCL injury and get back to the activities you love. You’ll also need time of limited activity to allow your body to heal safely. Your Axes physical therapist in Pevely, MO will be able to fit you for a knee brace in order to prevent your knee from bending too far inward while your MCL is still healing. With physical therapy, you’ll still continue icing your knee and taking anti-inflammatory pain medication.

Though uncommon, the most severe cases — in which the MCL is completely torn, the injury is accompanied by an ACL tear, or you are not recovering after several weeks of nonsurgical treatment — may require surgery. The surgery is performed by using a graft, either from one of your other ligaments or an organ donor, to reattach the torn sections. The surgery is an outpatient procedure. If you do require surgery in Pevely, MO, our physical therapists specialize in pre- and post-surgical rehabilitation to make your surgery and recovery as successful as possible.

The goal of physical therapy, when dealing with any injury, is to design safe, effective, and evidence-backed treatment methods based on each patient as an individual.
