How the MCL Works

MCL an acronym for “medial collateral ligament.” A ligament is like a strong, firmly attached rope that affixes bones to each other and keeps joints stable. The MCL connects your upper shin bone to the inner surface of the bottom thigh bone, running along your inner knee, and helps keep your knee stable and moving normally. Its functions to guard against valgus stress, or the knee being pushed unnaturally towards the other one. It is one of four ligaments in the knee, along with the:

  • ACL (anterior cruciate ligament), which runs down the center of your knee and controls rotation and controls forward movement
  • PCL (posterior cruciate ligament), which forms an “X” with the ACL in the middle of the knee and controls backward movement
  • LCL (lateral collateral ligament), which connects the femur to the fibula (the smaller bone in the lower leg) and gives stability to the outer knee

The knee is known for being one of our most complex joints. A healthy knee relies on surrounding bone and muscle, and the tendons and ligaments in order to provide stability and function properly. There are a variety of knee injuries that can greatly affect your mobility and ability to do what you love most, or just live a pain-free day-to-day life. Specific physical therapy techniques and other treatment modalities, prescribed by a doctor, have been demonstrated through research and studies to help patients reduce pain and return to normal functionality.

What Types of MCL Injuries Are There?

Generally speaking, an MCL injury is either a sprain, which stretches the ligament farther than is naturally, or a tear, where the MCL may fray partially or entirely separate from the bone. MCL injuries are “graded” in three levels:

  • Grade 1 — A grade 1 sprain or tear means that the ligament has been stretched, and less than 10% of the MCL has been torn. It’s typically a mild injury, and your knee largely remains stable. You will likely experience some tenderness and mild pain.
  • Grade 2 — This is more significant tearing, most commonly to the more fibrous part of the ligament. Your knee will be slightly unstable, and the knee will likely be loose when manipulated manually. Symptoms of a grade 2 MCL injury generally include much more noticeable pain.
  • Grade 3 — A grade 3 tear is a complete tear, or separation from the bone. Your knee will be significantly unstable. Pain will be intense and you’ll likely feel a wide array of symptoms. Grade 3 MCL injuries often occur alongside other knee injuries such as ACL tears.

Whatever grade MCL injury you have, the team of expert Villa Ridge, MO physical and occupational therapists at Axes can, along with your physician, develop a comprehensive and customized plan to get you back to the things you love without incurring further injury.

What Causes MCL Injuries?

40% of all knee injuries involve damage to a ligament, and most of the time that ligament is the MCL. MCL injuries occur due to the knee suddenly being forced too far inward. MCL injuries can also by the knee rotating more than the ligament can support. They can also happen because of landing or awkward bending, such as if your foot stays in one place while the rest of your body moves.

MCL injuries are common in athletes in contact sports and in sports that require a lot of sudden movement such as football, soccer, skiing, basketball, and tennis. They can also occur in people who lift weights or lift heavy objects at their work.

Common Symptoms of MCL Injuries

Most people feel pain immediately on the inside of the joint when they experience an MCL sprain or tear. Other symptoms may include:

  • A sudden “pop” on the inside of your knee
  • Instability
  • Not being able to put any weight on the knee
  • Bruising
  • Tenderness along the inside of the joint
  • Swelling, either of the MCL area or the whole knee

Sometimes your knee may feel like your knee is loose when you walk, even if walking doesn’t hurt, or in other cases you’ll barely be able to move the knee. If you experience any of the above symptoms, do not participate in any more strenuous activity and seek medical treatment for a diagnosis.

Diagnosing and Treating an MCL Injury in Villa Ridge, MO

Diagnosing an MCL injury requires going to a doctor’s office, emergency room or at a physical therapy center. The exam will usually start with a doctor or physical therapist in Villa Ridge, MO asking how the injury occurred. They’ll then bend your knee and apply slight pressure to the outside of it, noting your pain level and whether or not your knee is unstable. They’ll also gently feel for swelling, and may compare your injured knee to the healthy one.

Most physical therapists or doctors with knowledge of movement health in Villa Ridge, MO will be able to determine if you have an MCL injury with the above procedures. A phsycian may also order the following imaging tests to check the severity of the injury and whether or not you have suffered other damage to the knee:

  • An MRI (magnetic resonance imaging), which uses a magnetic field and computer-generated radio waves to create detailed images of the inside of your knee
  • An ultrasound, which uses sound waves and can help determine how severely the MCL is torn
  • An X-ray, which, while it won’t show the MCL or other ligaments, to insure your femur, kneecap, tibia, and fibula have not fractured

You can also visit the Axes location most convenient for you for a free, no-risk injury screening. While we can determine if you have suffered an MCL sprain or tear, Missouri law mandates that physical therapy be prescribed by a doctor.

Treating an MCL Injury in Villa Ridge, MO

Typically, treating an MCL injury begins with basic first aid: reducing swelling, reducing pain, and utilizing the R.I.C.E. Method of Rest, Ice, Compression, and Elevation. For sprains or slight tears, the R.I.C.E method, over-the-counter pain relievers, and some time spent resting may be enough to get mostly back to normal.

Professional physical therapy is needed for more severe injuries to restore strength and optimal range of motion and get you back to the activities you love. You’ll also need time resting to allow your body to heal safely. Your Axes physical therapist in Villa Ridge, MO can fit you for a knee brace so that you don’t suffer further injury. With physical therapy, you’ll still continue icing your knee and taking anti-inflammatory pain medication.

Surgery is typically only needed with severe grade 1 MCL injuries, or if the MCL tear is accompanied by an ACL tear or other damage to the knee. During the surgery, a surgeon will use a graft, either from one of your other ligaments or from another person, to reconstruct the ligament. You would be able to go home the day of the surgery. If you do require surgery in Villa Ridge, MO, our physical therapists will be able to treat you with pre- and post-surgical rehabilitation to make your surgery and recovery as successful as possible.
