How the MCL Works

MCL an acronym for “medial collateral ligament.” A ligament is like a strong, firmly attached rope that affixes bone to bone and keeps joints stable. The MCL runs from the inside of your upper shin bone to the inside of your bottoms thigh bone and maintains stability in the inner knee. Its functions to withstand valgus pressure, or the knee being forced unnaturally inward. It is one of four ligaments that help keep the knee stable, along with the:

  • ACL (anterior cruciate ligament), which goes through the middle of your knee and provides rotational stability and prevents the tibia from sliding too far forward on the femur
  • PCL (posterior cruciate ligament), which forms an “X” with the ACL inside the knee and controls backward movement
  • LCL (lateral collateral ligament), which runs along the outside of the knee from the thigh bone to the fibula (the smaller bone the small bone behind the tibia) and gives stability to the outer knee

The knee is known for being one of our most complex joints. It depends on surrounding bone and muscle, and the tendons and ligaments in order to provide support and function properly. There are a variety of knee injuries that can greatly affect your mobility and compete like you’re used to, or just complete basic tasks pain-free. Particular physical therapy exercises and other physician-prescribed treatment plans have been proven to help those who have suffered an MCL injury manage their symptoms and return to optimal movement health.

What Types of MCL Injuries Are There?

In general, an MCL injury is either a sprain, wherein the MCL extends beyond its normal range of motion, or a tear, where the MCL is either partially or completely torn or separated from the bone. There are three grades to MCL injuries:

  • Grade 1 — A grade 1 sprain or tear means that the ligament has been stretched, and less than 10% of the the fibers in your MCL are torn. These injuries are typically not especially painful or debilitating, and your knee will still be stable. Your knee will feel tender, and you may also experience soreness.
  • Grade 2 — This is more significant tearing, usually to the superficial part of the ligament. You will probably lose some stability in your knee, and the knee will likely be loose when moved by hand. Pain is much more severe in a grade 2 MCL injury.
  • Grade 3 — This injury is a complete tear to both the fibrous and deep parts. You will likely feel like your knee’s about to buckle. Pain will be intense and you’ll likely feel a wide array of symptoms. Grade 3 MCL injuries often occur alongside other knee injuries such as ACL tears.

Whatever grade MCL injury you have, the team of professional Warrenton, MO physical and occupational therapists at Axes can, alongside your physician, implement an extensive and personalized plan to get you back to the things you love safely.

MCL Injuries: Causes and Symptoms

40% of all knee injuries involve damage to the MCL, LCL, ACL, or PCL, and most of the time that ligament is the MCL. MCL injuries occur due to the knee suddenly being forced too far inward. MCL injuries can also occur if the knee is rotated more than the ligament can support. They can also take place when landing or excessive bending, such as if your foot stays in one place while the rest of your body moves.

MCL injuries usually occur athletes in contact sports and in sports that require a lot of torque being placed on the body such as football, soccer, skiing, basketball, and tennis. They can also occur in people who lift weights or lift heavy objects at their work.

Common Symptoms of MCL Injuries

If you suffer an MCL injury in Warrenton, MO, you’ll likely notice the pain right away. Other symptoms may include:

  • A sudden “pop” on the inside of your knee
  • The knee being unstable
  • Inability to bear weight
  • Contusions
  • Tenderness along the inside of the joint
  • Inflammation, either where the MCL is located or the entire knee joint

Sometimes you may feel a “wobbly” sensation, or in other cases the knee feels too stiff. If you suspect you may have suffered an MCL injury, cease any strenuous activity immediately and schedule a physical examination in order to get a diagnosis.

Diagnosing and Treating an MCL Injury in Warrenton, MO

Diagnosing an MCL injury requires going to a doctor’s office, emergency room or at a physical therapy center. The examination will usually start with a doctor or physical therapist in Warrenton, MO asking how the injury occurred. They’ll then bend your knee and apply slight pressure to the outside of it, noting your pain level and if your knee feels structurally sound. They’ll also gently feel for swelling, and may compare your injured knee to the healthy one.

Most physical therapists or physicians with knowledge of movement health in Warrenton, MO will be able to diagnose an MCL injury with the above procedures. A phsycian may also order the following imaging tests to check the severity of the injury and whether or not you have suffered other damage to the knee:

  • An MRI (magnetic resonance imaging), which uses a magnetic field and radio waves to create three-dimensional images of the inside of your knee
  • An ultrasound, which uses sound waves and can be helpful in checking the severity of your injury
  • An X-ray, which, while it won’t show the MCL or other ligaments, may be ordered for a severe MCL injury to ensure there are no broken bones

Axes Physical Therapy also offers injury screening injury screenings at no cost or obligation to you. While we can determine if you have suffered an MCL injury, Missouri Revised Statute §334.506 requires a prescription in order to officially begin physical therapy treatment.

Treating an MCL Injury in Warrenton, MO

Right after your MCL injury, you should start with basic first aid: inflammation reduction, managing discomfort, and using the R.I.C.E. Method of Rest, Ice, Compression, and Elevation. For sprains or slight tears, the R.I.C.E method, pain and swelling relievers, and a few days or week or two of rest may be enough for your body’s natural healing processes to get you back on your feet.

More significant tears will require professional physical therapy in order truly recover from your MCL injury and get back to the activities you love. You’ll also need to refrain from most physical activity for an even longer period. Your Axes physical therapist in Warrenton, MO can fit you for a knee brace in order to prevent your knee from moving side to side while your MCL is still healing. Even with physical therapy, you’ll likely still continue the R.I.C.E. method and anti-inflammatory medication.

Surgery is typically only needed with severe grade 1 MCL injuries, or if the MCL tear is accompanied by an ACL tear or other damage to the joint. The surgery is performed by using a graft, either from one of your other ligaments or an organ donor, to reconstruct the ligament. The surgery is an outpatient procedure. If your doctor thinks surgery is necessary, our physical therapists will be able to treat you with pre- and post-surgical rehabilitation, which ensures that your knee will be healthy enough for the ligament reconstruction, and you’ll safely restore, strength, flexibility, and range of motion after.
