What is the MCL?

MCL stands for “medial collateral ligament.” A ligament is a fibrous of connective tissue that connects bones to each other and helps hold structures with the body together. The MCL runs from the inside of your upper shin bone to the inside of your bottoms thigh bone and helps keep your knee stable and functioning normally. Its functions to withstand valgus pressure, or the knee being pushed too far towards the other one. It is one of four ligaments that help keep the knee stable, along with the:

  • ACL (anterior cruciate ligament), which goes through the middle of your knee and provides rotational stability and controls forward movement
  • PCL (posterior cruciate ligament), which crosses the ACL in the middle of the knee and prevents excessive posterior translation of the tibia relative to the femur
  • LCL (lateral collateral ligament), which connects the thigh bone to the fibula (the smaller bone the small bone behind the tibia) and gives stability to the outer knee

The knee is known for being one of our most complex joints. It depends on surrounding bone and muscle, and the tendons and ligaments in order to provide support and function properly. When any knee injury, such as an MCL tear, occurs it can limit or completely restrict your mobility and ability to do what you love most, or just live a pain-free day-to-day life. Specific physical therapy techniques and other treatment modalities, prescribed by a doctor, have been proven to help those who have suffered an MCL injury mitigate pain and swelling and get back to normal movement health.

What Types of MCL Injuries Are There?

Generally speaking, an MCL injury either involves a sprain, which stretches the ligament farther than is naturally, or a tear, where the ligament is either partially or completely torn or separated from the bone. MCL injuries are “graded” in three levels:

  • Grade 1 — A grade 1 sprain or tear means that the ligament has been stretched, and less than 10% of the the fibers in your MCL are torn. It’s typically a mild injury, and your knee largely remains stable. Your knee will feel tender, and you may also experience soreness.
  • Grade 2 — This injury is when the MCL suffers more significant tearing, generally to the superficial part of the ligament. You will probably lose some stability in the joint, and your joint may be loose when moved by hand. A grade 2 injury will result in significantly more pain than a grade 2 MCL injury.
  • Grade 3 — A grade 3 tear is a complete tear, or separation from the bone. You will retain little to no stability. Pain will be intense and you’ll likely feel several other symptoms. Grade 3 MCL injuries often occur alongside other knee injuries such as ACL tears.

However significant your MCL injuries, our expert Champ, MO physical and occupational therapists at Axes can, in concert with your doctor, develop an exhaustive plan uniquely made for you and your injuries to get you back to the things you love in a safe and effective manner.

What Causes MCL Injuries?

40% of all knee injuries involve damage to a ligament, and most of the time that ligament is the MCL. When you suffer a forceful trauma to the outside of your knee, it can push the knee inside far enough that the MCL strains or tears. MCL injuries can also by the knee rotating too forcefully. They can also take place when landing or unnatural bending, such as if your foot stays in one place while the rest of your body moves.

Athletes in contact sports are the most likely to sustain an MCL injury. They are also common in those who have to hunch over and lift heavy objects, either for athletic training or in the course of their employment.

Common Symptoms of MCL Injuries

Most people feel pain immediately on the inside of the joint when they experience an MCL sprain or tear. Other symptoms may include:

  • A sudden “pop” on the inside of your knee
  • Feeling like the may buckle
  • Inability to bear weight
  • Bruising
  • Tenderness where the MCL is located
  • Swelling, either where the MCL is located or the entire knee joint

Sometimes you may feel a “wobbly” sensation, while other times you’ll barely be able to move the knee. Any time you feel pain or instability in your knee, cease any strenuous activity immediately and seek medical treatment for a diagnosis.

Diagnosing and Treating an MCL Injury in Champ, MO

Diagnosing an MCL injury requires going to a physical examination. In the examination, your Champ, MO doctor or physical therapist will go over what led to the injury, will feel for swelling and how your knee is different from your healthy knee, and perform a valgus stress test.

Most physical therapists or physicians that specialize in the musculoskeletal system in Champ, MO will be able to diagnose an MCL injury with a physical examination. A phsycian may also order the following imaging tests to check the severity of the injury and whether or not you have suffered other damage to the knee:

  • An MRI (magnetic resonance imaging), which uses a magnetic field and radio waves to take detailed images of the knee joint
  • An ultrasound, which uses sound waves and can help determine how severely the MCL is torn
  • An X-ray, which, while it won’t show the MCL or other ligaments, may be ordered for a severe MCL injury to ensure there are no broken bones

Axes Physical Therapy also offers injury screening injury screenings at no cost or obligation to you. While we can offer you an evaluation, Missouri Revised Statute §334.506 mandates that physical therapy be prescribed by a physician.

With physical therapy, you’ll still continue icing your knee and taking anti-inflammatory pain medication.

Though uncommon, the worst grade 1 tears — in which the MCL is completely torn, the injury is accompanied by an ACL tear, or you are not recovering after several weeks of nonsurgical treatment — may require surgery. During the surgery, a surgeon will use a graft, either from your hamstring or from another person, to reconstruct the ligament. You would not have to be hospitalized overnight.
