How Does the Shoulder Work? | Shoulder Pain in Wilbur Park, MO

The shoulder is one of the the largest and most intricate joints we have. In actuality, it is made up of four independent smaller joints:

  1. The glenohumeral (GH) joint is the joint where the humerus (upper arm bone) fits into a relatively small dish called the glenoid. Given the relatively shallow dish, it is the most mobile joint in the body.
  2. The acromioclavicular (AC) joint is formed by the junction of the collarbone the acromion, the bony tip on the outer edge of the shoulder blade. A plane style synovial joint, it helps extend the upper extremity from the trunk.
  3. The sternoclavicular (SC) joint is a plane-style synovial joint located where the collarbone meets the sternum at the foundation of the neck. As the only joint that connects the arm to the body, it coordinates the movement of the upper limb with the core of your body.
  4. The scapulothoracic (ST) “joint” is not a true anatomical joint as it does not have ligaments connecting it to bone. It is made up of the scapula and its articulation with the ribcage.

In order to support those four joints, the shoulder consists of:

  • the four muscles of the rotator cup covering the head of the humerus, which are responsible for movement and stabilize the ball of the humerus within the shoulder socket
  • the shoulder capsule, a a fluid-filled sac that surrounds and lubricates the shoulder
  • the four shoulder ligaments which provide passive stability while still allowing for movements in different planes
  • the tendons in the biceps and shoulder which connect muscle to bone
  • the brachial plexus nerves that allow for the brain and muscles to communicate with each other
  • the blood vessels that supply nutrient-rich blood to the muscles, ligaments, and tendons

In order for the shoulder to function properly the entire complex system must be able to work together. Sufficient blood must reach to the shoulder efficiently to keep oxygen levels and muscles strong and repair minor damage. The muscles must have adequate strength and flexibility, the tendons attaching bone and muscle must be strong, flexible, and free of rips or tears, the ligaments connecting bones need to help support proper structure, and messages most be able to be sent and received between the brain and shoulder. When any one of the parts of the shoulder is damaged, inflamed, or otherwise irritated, it can lead to a disruption of the whole complex, and anywhere from nagging to debilitating shoulder pain or loss of function. If if shoulder pain or poor shoulder movement is part of your daily life, contact Axes’ physical therapists for a no-risk, no-obligation injury screening.

What Causes Shoulder Pain?

Shoulder pain can be caused by a variety of circumstances, from sports injuries to being struck by an object, a work injury from overuse to our bodies just getting weaker as we get older. The following are common causes of shoulder pain that our team see in Wilbur Park, MO:

  • Bursitis The bursae are small, fluid-filled sacs that cushion bones, tendons, and muscles in and around your shoulders, elbows, hips, knees, and other joints. Bursitis occurs when the bursae because swollen after repeated activities, often occurs in combination with shoulder impingement, and can make basic tasks like brushing your teeth tremendously painful.
  • Tendinitis Tendinitis is typically caused by a tendon becoming irritated. There are two types of tendinitis:
    • Acute: The result of a sudden blow, excessive overhead motions, or other overhead activities needed in some sports and fields of employment, such as volleyball or moving objects overhead.
    • Chronic: The result of wear and tear or general weakness as we age.

The condition causes pain and tenderness just outside the joint.

  • Rotator Cuff Tear These can also be caused by sudden trauma, such lifting an incredibly heavy object, or more chronic in nature. They usually start with fraying and then tear. A partial thickness tear often goes unnoticed. Full-thickness tears, which go all the way through the tendon, do not heal by themselves. Associated symptoms are having difficulty raising your arm or lifting things you normally can, pain when lying on the side of the affected shoulder, and a grinding sensation when performing certain motions.
  • Shoulder Impingement Impingement occurs when your acromion (outer edge of your shoulder blade) irritates the rotator cuff and bursae. It is often the result of rotator cuff swellings after an injury. Shoulder impingement is rarely caused by overuse, but rather by physical activity.
  • Shoulder Instability This occurs when the humerus is forced out of the shoulder socket, and also can be the result of injury or wear and tear. Dislocations can be repeated and partial or the entire bone coming out of the socket. It often is accompanied by injury to the supporting soft tissue of the shoulder. Chronic instability will cause shoulder pain and unsteadiness when performing certain motions.
  • Arthritis While there are at least six kinds of arthritis, the shoulder most typically experiences osteoarthritis. Osteoarthritis is the degeneration of joint cartilage and the underlying bones. Common causes include old age, repeated trauma, and wear and tear. There is no cure for arthritis, but you can find effective pain management methods with Wilbur Park, MO physical and occupational therapists.
  • Shoulder Fractures The upper arm bone, clavicle, and scapula are the bones in the shoulder most prone to breaking. They are often the result of falling a significant distance or onto a hard surface or high-energy collisions such as being tackles in a football game. The shoulder pain is immediate and obvious, along with substantial swelling and contusions.
  • Axillary Nerve Injuries or Brachial Plexus Injuries Nerve damage in the shoulder is the result of when nerves are impinged on, when scar tissue blocks their electric signals, or the nerves rupture. Axillary nerve and brachial plexus injuries can cause shoulder pain, stingers, weakness, and loss of feeling in your shoulder, arm, or hand. They are usually the result of sudden trauma but can also happen because of overuse.

Sometimes, injuries are connected, for example a dislocation that leads to damage to the nerves.
