How Does the Shoulder Work? | Shoulder Pain in Potosi, MO

The shoulder is among the largest and most complex joints we have. In truth, it is made up of four independent smaller joints:

  1. The glenohumeral (GH) joint is the joint responsible for connecting the upper extremity to the trunk. Given the relatively shallow dish, it allows for more movement than any other joint.
  2. The acromioclavicular (AC) joint connects the collarbone to the acromion, the bony tip on the outer edge of the shoulder blade. A plane style synovial joint, it helps extend the upper extremity from the trunk.
  3. The sternoclavicular (SC) joint is a gliding joint located where the collarbone meets the sternum at the foundation of the neck. As the only joint that connects the arm to the torso, it it ensures your arm and body act in tandem.
  4. The scapulothoracic (ST) “joint” is not a true anatomical joint as it does not have typical joint characteristics. It is made up of the shoulder blade and its articulation with the ribcage.

The shoulder is supported and surrounded by:

  • the four muscles of the rotator cup that surround the head of the humerus, which power movement and stabilize the ball of the humerus within the shoulder socket
  • the shoulder capsule, a soft tissue sac that encloses the shoulder
  • the ligaments of the shoulder which provide passive stability while still allowing for a wide range of movements
  • the tendons in the biceps and shoulder which allow muscles to power the bones
  • the brachial plexus nerves that allow for the brain and muscles to communicate with each other
  • the blood vessels that pump much-needed blood to the muscles, ligaments, and tendons

In order for the shoulder to function properly every joint, ligament, tendon, muscle, artery, capsule, and nerve must function well. Sufficient blood must flow to the shoulder efficiently to keep oxygen levels and muscles strong and repair minor damage. The muscles must have adequate strength and flexibility, the tendons connecting them to the bones must be strong, flexible, and free of rips or tears, the ligaments connecting bones need to help maintain structure, and the nerves need to have a strong and secure connection to the brain. When any of these functions are not being carried out in a healthy way, it can lead to a breakdown of the whole network, and anywhere from nagging to intense shoulder pain or loss of function. If if shoulder pain or poor shoulder movement is part of your daily life, contact Axes’ physical therapists for a no-risk, no-obligation injury screening.

What Causes Shoulder Pain?

Shoulder pain can be caused by a variety of circumstances, from sports injuries to acute trauma, a work injury from overuse to our bodies just getting weaker as we get older. The following are the most common underlying conditions that lead to shoulder pain in Potosi, MO:

  • Bursitis The bursae are small, fluid-filled sacs that cushion bones, tendons, and muscles in and around your shoulders, elbows, hips, knees, and other joints. Bursitis occurs when the bursae because swollen after repeated activities, often occurs in association with shoulder impingement, and can make daily tasks like brushing your teeth exceedingly difficult.
  • Tendinitis Tendinitis is typically caused by a tendon becoming irritated. There are two causes of tendinitis:
    • Acute: The result of a sudden blow, excessive throwing, or other overhead activities needed in some sports and fields of employment, such as tennis or lifting and lowering boxes.
    • Chronic: The result of wear and tear or the body becoming weaker as we get older.

Symptoms include aching, tenderness, and mild swelling.

  • Rotator Cuff Tear These can also be acute, like falling on an outstretched arm, or more chronic in nature. They usually start with fraying and then tear. People often live with partial tears unknowingly. Complete tears, where the tendon is no longer attached whatsoever to the bone, often require surgery. Symptoms include having difficulty raising your arm or lifting things you normally can, pain when sleeping, and a cracking sensation with certain motions.
  • Shoulder Impingement Impingement describes when your acromion (outer edge of your scapula) pinches the soft tissue in your shoulder. It often occurs in tandem with an injury that causes inflammation in the rotator cuff. Shoulder impingement is rarely caused by overuse, and is most common in athletes or others who engage in physical activity.
  • Shoulder Instability This occurs when the ball of the upper arm bone is forced out of the shoulder socket, and also can happen suddenly or from wear and tear. A dislocation can be partial, coming in and out of the socket, or complete when the bone completely comes out of the socket. It often is accompanied by injury to the supporting soft tissue of the shoulder. Chronic instability will result in shoulder pain and a sense of the shoulder “giving way.”
  • Arthritis While arthritis refers to a range of diagnoses, the shoulder most commonly experiences osteoarthritis. It is the result of the cartilage cushioning the ends of your bones degenerates. It can be the result of tissue weakening as we become older, repeated injuries, and wear and tear. Arthritis cannot be cured, but you can find effective pain management methods with Potosi, MO physical and occupational therapists.
  • Shoulder Fractures The upper arm bone, clavicle, and shoulder blade are the bones in the shoulder most susceptible to breaking. They are often the result of falling from a height or high-energy collisions such as a car accident. The shoulder pain is immediate and obvious, and the shoulder will experience considerable inflammation and contusions.
  • Axillary Nerve Injuries or Brachial Plexus Injuries Nerve damage in the shoulder is the result of when nerves are impinged on, when scar tissue blocks communication between the brain and shoulder, or the nerves rupture. These injuries can cause shoulder pain along with a range of other sensations. They are usually the result of high-impact trauma but can also happen because of wear and tear.

At times, one injury leads to another, for example how an untreated rotator cuff tear can lead to arthritis.
